How about using the rating card below? From a numbering from 1 (lowest) to 5 (highest) rating. So, a typical rating would look like the following;

Facilities 3 Bed 3
Ambience 3 Cleanliness 3
Service 4 Room Size 3
Location 5 Amenities 3
Value for Money 4
